Invitation of Applications for Private Practice of following Specialists at CB Model Dispensary, Walton Cantonment Board

| 3 | Radiologist | • FCPS/MCPS in Radiology |
| 4 | Gynaecologist | • FCPS/MCPS Gynae |
Terms & Conditions:
- The Consultant will be provided with a consultancy room for patients.
- The consultation fee will be Rs.1,000/- (per visit).
- Consultation will have 2 x types of consultation (Simple Checkup & Procedural consultation). The consultation fee shall be distributed in proportion between Consultant & W.C.B (Negotiable).
- Beneficiary patients / Entitled patients (i.e. Employees & Dependents of Walton Cantonment Board / Lahore Cantonment Board, RHQ ML&C and MEO, Lahore etc) shall only be entitled to consultation on subsidized rates (i.e Rs. 400/-), if they have been properly referred to the consultant on their health card.
- OPD days may be decided with consultant’s consent.
- Share of consultation shall be distributed on weekly basis and record of patients will be maintained on register/computer by the CGH.
